Healthdome.net may provide prizes in the form of Awards, Raffles, or Give Aways to Member or Non-Members over the age of thirteen. In order to claim any Incentive we require enough information to send you the prize.  Campaign Members will be listed on a Roster and disbursements will go through the Campaign Manager. Providing us with personally identifiable information is optional for Campaign Members, except for the Campaign Manager.  Members may use a "handle" or a "screen name" on the Roster instead of their real name.
